The topic is ‘Planning for your future’

Freedom of choice, making decisions, voicing your needs, hopes, wishes are all things we take for granted. However, life may change through illness, disability or life events that make this no longer possible.

Advance care planning is a process that allows you to discuss and document your hopes, feelings, wishes and preferences. It allows you to retain some control, allows you to make decisions, even if you are not conscious and have no voice.

It supports your loves ones, knowing they are supporting you within your values and wishes, assisting Health Professionals within your care.

The aim of this workshop is to introduce you to Advance Care Planning – allow opportunity for discussion and introduction to documenting your preferences and wishes for your future care. The wishes you may express may be personal to you from where you would like to be cared for, who you would prefer to care for your pet, to your preference of shower to bath.
