Make a referral
All health and social care professionals can make a referral to St Clare Hospice. Individuals can also refer themselves, or a close relative or friend, to St Clare Hospice.
Please note: when the patient is already an inpatient elsewhere, we will liaise with the hospital palliative care team.

Please send any referrals for Hospice care, and end of life care, to Essex Partnership University Trust (EPUT) Care Co-ordination Centre. Email: epunft.carecoordinationcentre.west.2023@nhs.net or call: 0300 123 5433. Please ensure that all referral emails are also cc’d to St Clare Hospice at nee.fcs@nhs.net to ensure a swift response.
The Care Co-ordination Centre will liaise with all providers to support each referral. The Care Co-Ordination Centre operates from 08.00 – 21.00hrs
