One patient, One record – St Clare goes online!

On 28 February St Clare Hospice launched the use of their new electronic patient record system, SystmOne. St Clare Hospice’s Inpatient Unit Manager, Vanna Poulter, who is overseeing the implementation of the new system, said: “We are delighted to be launching this new system at St Clare which will vastly improve our communication with staff from across the healthcare sector.”

Vanna continued: “It has been a long held aim of the hospice to adopt an electronic patient record system which integrates with the wider healthcare sector across West Essex and means that we can offer seamless care to those people with life-limiting and terminal illnesses. The system will enable us to keep uptodate with our patient’s treatment and progress – helping us to deliver timely, individualised care to each and every person who needs us.”

GP Surgeries using SystmOne will now be able to make referrals to St Clare Hospice via the SystmOne referral procedure. Those practices not on SystmOne can still make referrals through the St Clare website where a form can be downloaded, completed and emailed here.

We look forward to this new venture for St Clare but if you have any queries please do not hesitate to contact Vanna Poulter, IPU Manager, who leads our referral meetings, via the NHS email [email protected]
